Outside Activity Questionnaire Form

All staff planning to attend a conference sponsored by a non-state entity must complete and submit the State Ethics Commission form: Request for Approval for Attendance at Events. This includes conferences co-sponsored by the State where one of the co-sponsors is a non-state entity.

The form is available through the "Ethics" link in the left column on the DCF portal. It must be downloaded, completed, signed by the participant and his/her supervisor, and submitted to DCF Ethics Liaison Officer, in the Office of Legal Affairs, DCF or by email to ethics@dcf.nj.gov.

Please note:

• Forms submitted without supervisory signatures cannot be processed.
• Timely submission in advance of the event is important.
